Health form review and medical clearance
Customized prescription medication kits
Virtual urgent care support during travel
Emergency medication guidance

3. Eligibility and Geographic Restrictions

Eligible Users:

Must be 18 years or older
Must be residents of Arizona, Colorado, Iowa, Massachusetts, Nevada, New Mexico, Oregon, Utah, or Washington DC
Must provide accurate health information

We do not provide services to:

Minors without parental consent
Residents outside our licensed states
International customers

4. Medical Clearance Process

Required Steps:

Complete health form through Charm Personal Health Record
Provide accurate medical history and current medications
Await medical review by Sara Levin, FNP-C
Respond to any follow-up questions if contacted

You agree to:

Provide complete and accurate health information
Update your health information if it changes
Respond promptly to medical clearance questions

5. Prescription Medications

Important Acknowledgments:

All medications are prescription-only and require medical clearance
Medications are prescribed specifically for you based on your health profile
You may not share, sell, or distribute medications to others
You are responsible for proper storage and handling
You understand medication instructions and contraindications

6. Payment and Pricing

Payment Terms:

Payment is due at time of order
Shipping costs are included in kit pricing
All sales are final - no refunds due to FDA regulations
Prices subject to change without notice

7. No Refund Policy

Due to FDA regulations and safety requirements:

No refunds once medications are shipped
No returns of prescription medications accepted
All sales are final to ensure medication safety and regulatory compliance

8. Shipping and Delivery

Shipping Terms:

Processing time: 1-2 weeks after medical clearance
Adult signature required for delivery
Customer responsible for providing accurate delivery address
Shipping included in kit price

We are not responsible for:

Delivery delays due to weather or carrier issues
Packages lost after confirmed delivery
Incorrect addresses provided by customer
